Practitioners Raise Consistency of Patient Care through Teams
The team-based model of care employed by Your Health meets needs by actively engaging patients and having their providers practice to the full extent of their education, certification, and experience. Nurse practitioners working with a partner alternating weekly twelve-hour, seven-day schedules can offer patients a more consistent level of care. Teamwork is also conducive to better coordination and collaboration on available treatment options.
The average lead time for an appointment in a traditional doctor’s office is approximately three weeks. With Your Health, having practitioners available seven days per week and seeing the patient in their home reduces that appointment window to just a few days. The faster appointment times are often critical for helping patients avoid more serious conditions that might land them in the hospital. In addition to quicker appointments, practitioners can see patients on a more regular schedule, which is a critical component for those suffering from a chronic illness.
By designing primary care services emphasizing prevention, health promotion, and chronic disease management, Your Health seeks improved outcomes throughout its patient demographics. The team approach lends itself well to proactive care where expertise and resources are customized for the individual patient. Continuity of care is further enhanced with in-house specialists who currently include over ten specialty services available for consultation or in a Telemedicine format for the patient.
