RPM kits can be placed in a patient’s homes to monitor a patient’s healthcare status.

Your Health has partnered with Clinii to provide RPM kits that can be placed in a patient’s homes to monitor a patient’s healthcare status. The RPM kit helps manage chronic disease and acute illnesses in the patient’s home using sensors and blue tooth enabled FDA approved medical devices.

Each kit is specifically created to fit the patient's diagnosis. Kit can contain the following: -Clinii Mobile which uses AT&T data with capabilities that allows transmission of vital signs to our web-based portal for symptom-specific assessment by our providers. -Pulse Oximeter- accurately measures your Pulse Oximetry and Heart Rate -Digital Scales for patients up to 400lbs -Automated Blood Pressure Cuff custom designed for one-hand positioning - AccuCheck Glucometer with strips, lancets, and injector pen. All devices are blue tooth enabled and are cordless to decrease the risk of trip hazards in the home.

The RPM kit helps manage chronic disease and acute illnesses in the patient’s home using sensors and blue tooth enabled FDA approved medical devices.

  • Patients that have chronic illnesses such as Heart Failure, Chronic Obstructive Pulmonary Disease, Hypertension and Diabetes are great candidates to utilize RPM. Also, patients with a history of falls and readmissions to the hospital, as well as COVID-19 positive patients would greatly benefit from the use of RPM.

  • Contact Your Health at 1-800-491-0909 to schedule an appointment with one of our providers. The provider will come to your home for a visit or perform a telemedicine visit, to review your past medical history, current diagnosis, current medication list, and insurance carrier to determine if you meet eligibility for RPM.

  • We generally recommend a 60-90 window for teaching and increased compliance with diet and medication compliance for using RPM, but it can continue past this window.

  • Over the last year, Your Health has provided RPM services to over 675 patients. Within the last three months, we have seen a less than 4% readmission rate to the hospital for our patients under the RPM program. This is incredible, considering that nationally the benchmark is 16% for patients under RPM programs. We feel using RPM directly impacts patients by improving their quality of life and overall physical, emotional and mental health.

  • We highly encourage patients to use RPM at a minimum ONCE DAILY, but you can also use it as many times during the day as you would like. Our recommendation is to take your morning medications an hour prior to monitoring each morning, preferably by 10am. Also, if you become ill throughout the day, it would be a good time to utilize RPM to note vital signs and see if you need an acute visit with a provider as well.

  • RPM devices can be set-up anywhere in the home. We recommend setting the scale up on a hard floor surface, not on carpet, as this effects the results when using the scale. An electrical outlet should be close by in order to keep the tablet plugged in at all times.

  • Your Health Providers as well as Community Health Workers can be reached in the event of needing support with RPM Equipment.

  • Patients only need to be able to follow clear auditory and visual cues and safely stand if weights are needed to use the RPM equipment. The data from the equipment is automatically downloaded into the patients chart. The RPM system is flexible and customizable- not all pieces of equipment must be used by each patient.

  • Currently, RPM is a Medicare Program. Primary insurance should can be any kind of Medicare. [Those would include: Traditional Medicare, Medicare Advantage, & Medicare Replacement plans.] Patients under the RPM program would still be responsible for any deductibles or co-insurances related to this service. If patients receive a bill, they can reach out to their Community Health Worker who will get in contact with the Business Office Manager for their region to assist with their bill.
